Request for Research Surveys

  1. Detailed plan of the research activity and rationale for conducting the study
  2. IRB approval or letter from the IRB committee indicating that approval is not needed
  3. Letter to the SCCM membership to accompany the survey request
  4. Survey tool questions in either Microsoft Word or Excel spreadsheet
  5. Link to the actual online survey
  6. Specifications to whom this survey needs to be directed (e.g., physicians, fellows, residents, nurses, respiratory therapists, pharmacists, an SCCM specialty section, etc.)
  7. How you plan to disseminate the survey to SCCM members
    • SCCM Direct Email Service
    • SCCM Research Website with announcement in the -Newsletter (surveys are posted for 3 months)
  8. For more specific information on the designing a survey tool please review A Guide To Surveys power point presentation produced by the SCCM Research Committee.

After SCCM receives the requested information, it is forwarded to the Research Committee for review.  The review process can take 2-4 weeks.  After review of the request, the Research Committee will make a recommendation to the Executive Committee. If the Executive Committee grants approval, the introductory letter and a link to the survey tool will be disseminated to the specified membership group as indicated in request form.
