*Fellows, residents and students register at te Healthcare Professional rate. Training verification letter must accompany registration form for fellows, residents and students.

Payment Options:
* American Express, Discover, MasterCard and Visa credit cards may be used to register online, or via phone, fax or mail.
* Check or international money order (payable to SCCM) may be used to register via mail ONLY
* Fees must be paid in U.S. funds and checks MUST be drawn on a U.S. bank

Cancellation/Refund Policy:
Cancellations must be submitted in writing. All cancellations are subject to a $75 non-refundable processing fee, and must be postmarked prior to August 15, 2008 to be eligible for a refund. Any cancellation postmarked after this date will NOT be refunded. Exchanges and substitutions are not allowed at any time. Dates for the 2008 Clinical Focus: Venous Thromboembolism in the Critically Ill and Injured conference are subject to change and/or cancellation. In the even of a change/cancellation, only individual registration fees will be reimbursed. Please allow four weeks to process refunds.
